See below: Sperm morphology is poorly understood. According to the The World Health Organization guidelines, at least 4 percent should have a normal shape. Though normal fertility is poor with &lt;4%, success can still be achieved with IUI and IVF. The 3 main causes of poor sperm morphology:
* a genetic trait
* exposure to toxic chemicals
* increased testicular temperature 
The last 2 causes may be reversible.

Shoulderpain: Tough to say without examining you.  Your symptoms sound as if you at least strained one of your ligaments in your shoulder, with a possible co-existing Muscle strain of your scapula. Shoulder blade mass: This may be a ganglion cyst ( though shoulder blade area is rare) which can be painful  or a regular cyst/fibroma. if it is painful follow up with dermatology/surgery  as they can remove it .

This may be fine for various forms of little people but the measurements don't add up to anything expected. Rapidly increasing head size deserves a study of what is inside. Ultrasound may work at this age.

Yes: If the measurements you listed are accurate, you have a kid whose length is 85 % /weight  95% &amp; head size &gt;98% for age. By themselves, the measurements are not too unusual. However if the HC has crossed growth lines by this amount in 2 months a study of the skull contents is warranted. Ultrasound through the soft spot or a CT would help separate the problems from a simple large head.

how long does it take to feel any benefits of an increased dose of duloxetine going from 60mg to 120mg and if feel more medicated after starting the increased dose (head feeling 'buzzy') can this wear off over time?

Duloxetine: a potent: serotonin/norepinephrine reuptake inhibitor (SNRI) antidepressant. Common side effects: dizziness, somnolence, fatigue. Response is individual but in general doses above 60 mg may not add benefit and time to response may take 2-4 weeks. Side effects often resolve within that time otherwise, if intolerable, dose reduction or medication change may be necessary. Inform prescriber. Best regards.

my boyfriend has an issue with the skin around his penis tightening while erect, ihe can not become fully erect anymore as it used to, this also has increased the amount of time before he can perform again. any suggestions or exercises?

See a urologist.: It's not entirely clear from your question what may be happening, but i recommend evaluation by a urologist. If he is uncircumcised, a condition called phimosis where the foreskin does not retract properly could cause this skin tightening and impede erections. An full evaluation will be able to determine if phimosis, or some other dermatologic or infectious cause might be the problem.

my dr increased zispin by 7.5mg yesterday.today i have nagging pressure feeling in head.could this be increased anxiety. thanks?

?side effect: Tell your doctor about the "pressure feeling in your head." He/she may want to first rule out a side effect of increased Zispin dose and/or any drug interaction effect with Lexapro (escitalopram) and methotrexate which you're also taking.  Reversing the dose increase should answer the question. If pressure in head disappears, you've had a side effect. Please review with prescribing doctor. Best wishes.
