i had an endoscopy done, i was told i had a hiatus hernia,  so why do i need to go back to hospital to see a doctor. when i have to see my gp as well.

You have a choice: Hiatus hernia is often asscoaited with acid reflux. If you have this problem you will experience indigestion or hyperacidity. This can be manged by the endoscopy doctor as well as by your gp. But you will need good antacids which your gp may be bale to handle. Otherwise you can go see your gastro-enterologist who did the endoscopy because he is the specialist!.

have upper abdomin discomfort and pain, months now. gp suspects a hiatal hernia.  now noticed referred shoulder pain when press below the ribs centre of abdomin.  does this influence urgency of being seen?  referred to hospital but no appointment yet

Gallstones?: Have you had any imaging done?  Upper abdominal discomfort can be many things.  Gallbladder disease can present as you describe.  That is best evaluated with ultrasound.  CT scan may be helpful as well, and will identify a hiatal hernia.  The most common symptom of hiatal hernia is heartburn that's hard to treat.

: You don't mention what your symptoms are or whether PPI meds improve your symptoms. Your DeMeester score certainly is consistent with reflux but doesn't necessarily show that it's causing symptoms you may be experiencing. Erosive duodenitis may be the cause of symptoms. Aggressive acid suppression for a prolonged period.would.probably be required. Your gastroenterologist can advise you best.

Depends on the pain: The ED cannot do much for the hiatal hernia other than treat any heartburn pain - probably a GI cocktail to neutralize the acid. The Kidney stones are probably your bigger concern. Some people with kidney stones need to be treated as an inpatient with IV pain meds. Often kidney stones are treated consevatively for up to 72 hours with forced fluid intake and oral pain meds. Go to ED if pain severe.

Yes: I am not familiar with the hospitals/doctors in the bay area. However, i can comment on then 2nd part of your question. Yes, if there are no other contraindications laparoscopy or even robotic surgery is better than open surgery especially for hiatal hernia in an 83 year old. Quicker recovery, shorter hospital stay, less pain, less risk of infection.

Varies: Heartburn, food or acid reflux into the throat or mouth, food sticking in throat or chest with swallowing, vomiting blood, chest discomfort. Small hiatal hernias may have no symptoms.

Rare: Yes but very rare in children, rarely will see gerd , reflux problems,  but infants & newborn  will have  dangerous congenital diaphragmatic hernias.

Symptoms: Home treatment is treating symptoms. Do not eat 3 hours before bed time. Avoid chocolate, wine & sweets at night. Weight loss & exercise helps. Antacids or otc antacid medications (famotidine, etc). Sleep with head of bed slightly elevated using 2by4 board under the feet of the bed. See a doctor or gastroenterologist if no improvement. This info assumes you already have proper medical diagnosis.
