i'm on clarithromycin for tonsillitis but my hiatus hernia has flared up after 2 days a other doctor prescribed zithromax over the phone is it safe to use.

The two are similar: Tonsillitis can be from strep throat, a cold or influenza (antibiotics usually not used), or from some other cause such as allergies, irritants, or acid reflux. Usually, a person sees a doctor if the sore throat is bothersome, recurrent, or doesn't go away quickly. If getting stomach symptoms from strep throat treatment, one's doctor can recheck. Clarithromycin is very similar to Zithromax.

A 2cm hiatal hernia can certainly cause problems with eating, you certainly need to rule out other GI issues before having an operation that is irreversible. Continue working with your GI doctor, or ask for a referral to an esophageal specialist first to be sure.

Possibly: If you have pain behind your breastbone, it is important to make sure it is not caused by a problem with your heart. The burning sensation caused by gerd usually occurs after you eat. Pain from the heart usually feels like pressure, heaviness, weight, tightness, squeezing, discomfort, or a dull ache. It occurs most often after you are active.

What have you tried?: Your sizable hiatal hernia may be contributory to your symptoms.  Is it fixed, sliding, para-esophageal?  Before you consider surgery for the hiatal hernia and its manifestations of gerd, have you tried and failed appropriately-dosed acid blocker agents and anti-reflux diet modification?  Has a ph study and manometry been considered? Does your stomach empty normally?
