I am not sure what: The "black heads" are. However, there are montgomery glands on the areola surrounding the nipples. They are raised and you might be mistaking them for pimples. Their main function is lubricate the tissues.

Yes: You can get them wherever there's little follicles. Only a very large one or a very painful one will require medical attention.

my nipple has been itching on top for two days and underneath the nipple its a little bumpy and both of my nipples they always get little pimple like bumps at the end of my rings that i pop and puss comes out , why is my nipple so itching?

Pustules: These pustules or pimples are infections. It is unwise to intentionally pop them. Itching of nipples may be reactions to clothing or bra's. Your rings may be predisposing you to recurrent nipple infections. I recommend consulting a physician for treatment of the active infections and instructions on how to prevent them after eradication of the current infections. Apply antibiotic ointment.

Montgomery tubercles: Montgomery tubercles are glands on the nipple and aerola of the breast.  They are important in keeping the nipples lubricated during breastfeeding.  They are a normal part of the breast anatomy.  They are not whiteheads or acne.

See a dermatologist: Probable acne. This is a matter of genetics and hormones, not hygiene or stress or diet. Some birth control makes it worse, some makes it better. PCOS (irregular menses) may be a factor. A skin specialist will help you.

Systemic Rx: Acne on the trunk usually does not respond completely to topicals such as benzoyl peroxide. Your physician needs to see you and can write for a systemic antibiotic such as tetracycline which might in a low dose be enough to manage you. If not, Isotretinoin is a consideration.

Pustules: it sounds like it could be pustular or cystic acne, but I can't say for sure without seeing it up close.  I think you need to make an appointment with your primary care doctor, or a dermatologist if that is possible, ASAP. You will probably be prescribed antibiotics, and possibly Accutane if it is nodular or cystic acne. Please make an appointment first thing tomorrow. I hope that helps.

Length of time present, placement, color, flat to the skin or raised, irregular or regular margins, suspected triggering event, etc. all matter.
