Ears: We hear by 2 mechanisms: external sounds via air conduction and internal sounds by bone conduction. Normal anatomy: your carotid artery passes through the bone that makes up your inner ear. Bottom line: you likely have serous otitis media or an intermittently congested eustachian tube. When your external hearing is diminished, you hear the softer internal blood flow. This is not dangerous.

Normal.: It is normal to occasionally hear one's own heartbeat because the carotid arteries are very close to the ears and there is some 'internal travel' of the noise and sensation to the inner ear.

These symptoms could: be from wax buildup in the ear canal or from fluid buildup inside the ear behind the drum. Either way, you should be evaluated by a health care provider (preferably an ENT) to avoid potentially serious problems in the future.

Tinnitus: Not likely. A MRI and MRA of the brain is a good screen for an arteriovenous malformation (AVM). Tinnitus often occurs after exposure to loud noises. Pulsatile tinnitus can occur in dural AVMs but this occurs most frequently in women between 50-70 years of age. It is common to hear your heartbeat in your ears when lying down. See a neurosurgeon or neurologist to evaluate the symptoms.

Normal: Most people become aware of their heartbeat at night sometimes. This is not unusual. If you have a clogged ear (serous otits media), it will be even louder. If you turn left side down, it will be even louder.  If it bothers you, try white noise or music in your room at night. Or just ignore it. Think of a baby in its mother's womb listening to her heart: it's a relaxing & reassuring sound of life!
