: There is no form of birth control which is "100%".  With respect to "PCOS", spontaneous ovulation can occur, although not usually in a regular frequency, so even if one is using low dose oral contraceptives, there is a minimal chance of pregnancy, but not zero.

PCOS and Fertility: The combination of Insulin resistance and high testosterone is pcos can negatively affect ovulation and fertility.  This does not make you infertile.  Treatment of the condition with lifestyle changes like diet and exercise and medications like metformin improve the problem and increase the likelihood of conceiving.

Absolutely!: Many women with pcos have associated metabolic problems. In fact, best to address any health issues before becoming pregnant to maximize potential for healthy pregnancy and healthy baby.

Lose weight!: The only way without medication is significant weight loss.  Some women with pcos get pregnancy anyway, so there's always a chance -- but therapy such as metformin has a much higher chance of safe successful pregnancy.  Let your doctor help you - at your age, time is getting short.

Ovulation induction: Most women with pcos do not ovulate regularly.  The expedient way to achieve conception is with ovulation inducing medication.  Weight loss is also effective at restoring ovulation, as is surgical treatment of the ovaries.  See you doctor for a discussion of your options.
