Worms: Although Enterobius vermicularis (pinworm) can cause extreme itching -usually at night- most crawling sensations in the anal area are not parasitic. usually caused by pruritus ani, a condition generally caused from poor hygiene, or allowing soiling of the anal area to persist after bowel movements.

Food allergy or sensitivity could be the ticket here, but either yeast overgrowth can itch.

Pruritis Ani: Pruritis in not related to worms per say. It is related to ph issues of the stool, trauma from increased wiping and aggressive cleaning and even chemicals from some otc therapy. Try local care with balneol otc and a barrier cream and increase the fiber in your diet or some insoluble fiber supplement for long term prevention.

Inflammation: It seems you are having anal and peri-anal irritation. The commonest condition is pruritus ani which is irritation in the  perianal area due to incontinence or poor hygiene. A simple office examination can diagnose it and the treatment is usually keeping the area clean and mild topical corticosteroid creams.

Pinworm : Pinworm  is another name for enterobius vermicularis. It lives in the large intestine.  Pin worms are visible without magnification. They look like little white worms. The female is about the length of staple and the male is smaller.   During sleep female pinworms travel from the large intestine to lay their eggs around the anus.  
   Often a person with pinworm doesnâ€™t have any symptoms. There can be itching around the anus or vagina and in more severe cases there can be problems sleeping, sense of restlessness, abdominal discomfort and drop in appetite. 
  The easiest way to diagnose pinworm is using an adhesive tape from the doctorâ€™s office. Either the first thing in the morning or in the middle of the night, you apply the tape to your anus. Egg and sometimes worms will adhere to the tape which is then brought back to the doctor. The tape is microscopic evaluated to identify the worms.
