Likely trigger thumb: If the lump is on the palmar side of your thumb by the web space it is likely "stenosing tenosynovitis" or "trigger thumb"  this can cause clicking or locking.  It may respond to a steroid injection.  If not siimple release under local anesthesia is immediately corrective.

An echocardiogram detects abnormalities of heart valve and muscle,if any. Low blood pressure is often normal in young females but your physician takes a history,examines you and then orders appropriate tests.

Which thumb joint?: I'm not certain which joint but if it's the one closest to the fingertip this is osteoarthritis usually. Usually eventually gets better but you can try hot water, hot paraffin wax, isotonot gloves - that all my help.
