Yes: Your TB test is definitely very positive to TB and blood in sputum is always of concern.  At the least, i think you need a chest x-ray tofind out what is going on since a negative AFB in sputum does not always rule out active TB and there may be other conditions which can cause the blood in sputum.

Bronchoscopy: Ask for a bronchoscopy.  A flexible camera that looks at the inside of your lungs and upper airway.  Can be done by a pulmonologist or thoracic surgeon.

Viral process: Likely viral inflammation in the airway  with  some  blood flecks. You should see a physician and have a x-ray . You may need cough rx, anti-inflammatory therapy and possible antibiotics. If blood in sputum persists a bronchoscopy  and chest ct may be necessary.

See doctor...: The blood you coughed up could be related to the nose bleed. But, the fever makes acute bronchitis a likely diagnosis. Coughing up blood is called hemoptysis and the most common reason for it is bronchitis. You need to see your doctor for diagnosis and treatment because you may need antibiotic treatment.

on zpak for 5 daysfor sinus day 7 still blowing lots of orange and sometimes mixed with blood seems never ending feel good no fever no cough ?

Sinus infection: Of course you know it is a sinus infection. If bacterial, zpak usually works but you need to be on it for a couple cycles( usually 2 zpaks, 10 days.  If your not better at all you may need to be on a different antibiotic all together and usually for 3 weeks. Hard to say. Go see the md who put you on the zpak.

: A lung infection is likely, consider imaging of the chest. If you are a smoker, please try to quit. A sputum sample analysis for culture and cytology may provide additional information. Watch for signs of weight lo ss, monitor temperature and breathing. COVID-19 screening is recommended.

Although the: chances are slim that your symptoms are connected to 
the anal sexual contact (probably a more common stomach upset (viral or dietary origin) there are sexually transmitted diseases which can be transmitted anally..gonorrhea and non-specific urethrits are examples. If your partner wore a condom as he should have for his own protection, this would diminish that possibility. See your GYN !!

Sounds like my wife: Same age, complaints ; symptoms. She had tb! this was only cultured from concentrated pleural fluid. Being a diabetic some of the meds increase the risk of developing tb.
