Sounds accurate: however the etiology of the palpitations has not been defined. Are the palpitations caused by Afib, USVT, SVT, etc. Once defined then they can be treated so you will sleep better at night.

Decreased EF: Reduced LV function at your age is unusual, and may be due to issue with the heat muscle, infections can do this, certain viral infections are known. You should see a heart doctor to discuss the finding of the 2DE and ascertain as to why there is decreased function. A follow up 2DE should be considered at a certain interval. Best of luck.

Need cardiologist!: There are a number of concerning findings on your echocardiogram. The left ventricular function (how strong your pump is) is just a little low; I'm not that concerned about that. The valves, especially aortic, are the biggest problem: to have both aortic regurgitation (back flow across the valve) and narrowing (stenosis) is very concerning. Heart failure: Left side (ventricle) of heart pumps blood to body. Ejection fraction is a means to determine efficiency of heart at pumping out blood. Normal is greater than 50%. So anything less is left ventricular dysfunction aka heart failure. Best to control blood pressure, diabetes, cholesterol, etc. Stop smoking. NO: Nuclear stress test are done to evaluate coronary circulation/ blockages/CAD. As lagniappe they provide info re LV function. However, there are less expensive and invasive test that evaluate LV function just as well or better, like echocardiography
