Depends on symptoms: Briefs for men are like bras for women.
Briefs support the dependent structures (i.e. The scrotum)
if the scrotum is larger from a hydrocele (fluid collection around the testicle), when standing it may pull on the testicle and stretch the cord structures associated with the testicle- supporting the weight with briefs may be more comfortable.  If symptoms seek surgery/urology advice.

In men, this term is usually heard when a soft (sometimes uncomfortable) lump is felt in the scrotum. This requires medical evaluation to make sure it's nothing more serious.

Yes: Any surgery is going ot have some degree of pain no matter what. The level of discomfort depends on several factors including the size of the hydroceles (big ones tend to hurt more), the technique used by the surgeon and the pain tolerance of the individual.

Bottle or Lord ops.: For adult hydroceles. Both operations are performed through the scrotum. Excess hydrocele sac is excised with bottle procedure and remaining sac is everted and sutured together behind the testis. Hydrocele is incised and rolled up & sutured  on either side of testis with lord procedure. Infant & child hydroceles are corrected through the groin & patent communication, .If present, is divided.
