It's possible: Smooth fluid filled swelling o scrotum is usually a hydrocele. Worth seeing a urologist for an e4xamination & ? scrotal ultrasound. They are usually benign but sometimes can result from &/or mask a testicular tumor.

No, if ... : Clinically, a man would have NO hydrocele if the size of scrotum is not bothersome mentally and physically, and intrascrotal contents - testes / epididymides - are palpable bilaterally, you have no hydrocele and don't waste time to worry about it.
