Folliculitis: This is probably a small skin infection that started as folliculitis. I would not remove this lesion but would prefer a topical antibiotic and a warm compress several times per day. I would continue this for 3-5 days; these lesions typically dry up on their own but can leave a bruise on the skin for several months.

Not likely: It is highly unlikely that second hand smoke exposure would lead to a small fetal abdominal circumference. The question to ask your doctor is if the abdominal circumference is small but normal or small and abnormal indicating an overall problem  with fetal growth.

May be a hernia. : You should make an appt with a flesh and blood doc who can examine you directly, not a virtual Doctor. If you have not found someone to your satisfaction in Brooklyn then I have an office in Manhattan. It sounds like you might have either a direct or indirect hernia. If you indeed have that as the cause of your problem there is a good surgical fix for it!

Back mass: I don't know, may be from a lipoma to a sarcoma.
I know, you are worried, therefore you should see a Dr. he will examine you and provide an intelligent advice.
If you still are unhappy, request surgical removal,  if is not large can be done in the office.
The tissue will be sent for microscopic examination.
The results will be communicated to you, and further therapy done according the histology.
