friday i had a nose bleed that would not stop.  after 3 hours i went to urgent care. the dr put a cotton plug into my nostril.  bleeding stopped - can i remove the plug or should i have a dr do it?

Did : Did the urgent care doctor tell you if there was a visible bleeding vessel? Were you told to see an ENT (ear, nose, and throat specialist)? If not then the bleeding was probably due to dry nose secondary to allergies or other issue, however it would be wise to follow up with your primary care provider for a more clear understanding. You should be able to take the cotton out yourself. However if the bleeding persists and does not stop you need to go back to the urgent care.

on monday had a major nose bleed and went to er they put a plug and on wednesday they removed it but i'm spitting out blood with mucus is that normal ?

Yes: Yes it is with the plug in you are swallowing some of that blood that later on you will spit out

i feel shocks in my head/neck that feel similar to being shocked by a plug once removed from the socket but i feel this when doing every day activities. should i get this checked out?

Symptoms of this: nature are often a sign of neurological issues or spinal compression issues. It would be surprising to have these in a 20 year old but not impossible. Is It Seasonal?: If it is only in winter, it may be from dry air. Try using a humidifier at night and NaSal or a Netti Pot to irrigate and moisturize the nasal passages. If there is colored discharge, fever, or other symptoms, or it is year-round, talk to you physician about allergy testing.

why would someones nose plug up every nite, even while sitting, with no cold?  fine during the day.

Turbinate cyclings: Often the act of laying down results in swelling of turbinates.  Turbinates are radiator fins that run along the bottom of the nasal passageway and are highly suspectible to gravity.   Many patients tolerate swollen turbinates during the day because they tend to stay open when upright, however, can expand when lying down without the beneficial effects of gravity.  Consider steroid spray treatment.

pft done. normal range except dlco severely low. no nose plug used. could values be wrong? what does this mean? can that improve? prognosis?

Accepted Answer

Unusual: DLCO (if corrected with VA) reflects gas transmission through the lung tissue. It is unusual to have a totally normal lung function and severely low DLCO/VA. I would recommend that this be repeated or a HRCT chest be obtained to rule out interstitial lung disease if you have respiratory symptoms or unusual occupational exposure. Else it is more likely a lab error.

Saline nasal spray: may help soften and thin the mucous, and help to get it cleared out.   It can be used several times a day.  Be careful about using any other nasal sprays unless directed by a physician.   Mucinex (guaifenesin) (sold over the counter) can also be helpful in thinning and clearing nasal discharge.

why does blowing your nose cause plugged ears?

Plugged ears: This is a great question and it really addresses  the normal anatomy of the nose and ears.  The middle ear and nose are directly connected by way of a tube called the eustachian tube.  When you blow your nose, air is forced into the nose and up the eustachian tube.  This will cause an increase in pressure in the middle ear, which will quickly dissipate with swallowing.

Stuffy nose: Your symptoms are consistent with rhinosinusitis or inflammation/swollen of the nasosinus mucosa. In the US, the most common cause of it is allergy. Interestingly, when you lay down one side or the other, your body will try to cause vasoconstriction on one side of the nostril so you feel it open up a little. However, when the inflammation is severe, both will be plugged.

thick nasal mucus - i have to blow my nose very hard to get the plug feeling out, it is commonly very thick, white (not clear). should i be concern?

Thick nasal mucus: Thick nasal mucus can be related to sinusitis, but not necessarily. If you have chronic allergies or recurrent infections you should consult a doctor who can identify if there is an opportunity to improve nasal inflammation with sprays or treat infection with antibiotics. You may benefit from good nasal hygiene as well which involves using salt water rinses to clear mucus and allergens.
