Heat exercise massag: Moist heat applied to the neck & shoulders; pain med (aleve/advil/motrin—if tolerated by stomach & not on blood thinner coumadin); magnesium to help relax muscles (caution: too much may cause loose bm's); neck and shoulder exercises to relax and fatigue cramped muscles; therapeutic massage; consult doctor if problem worsens or persists more than a few days.

Need more info: Were you in an accident? Often neck pain can cause headaches often due to stress in the joints of the spine. This can also be aggravated in certain positions such as you describe. See you doctor, you may need x-rays, further an MRI may also be warranted if you symptoms persist. In the interim pt may also help.

Possible: You may have nerve root irritation. I recommend seeing someone experienced in diagnosing cervical spine issues. Inflammation  around a nerve root can cause these issues, including the sensations you are mentioning. Once an exam has been done, and possible imaging, you may need physical therapy, as well as other therapeutic modalities to calm the issue.

Need eval: Sounds like it could be arthritis or inflammation of the facet joints in the back portion of your cervical spine.  Start by seeing your pcp.  X-rays or an MRI may be in order.  You can take otc ndaids like motrin.

Exercise is best: Unfortunately painful discs in the neck and low back can be difficult to manage.  The only intervention that has been shown to help long term is a regular exercise regimen focusing on stretching and strengthening the back (core muscles), neck and shoulder stabilizers. Nicotine should also be avoided. Massage, chiropractic or traction treatments can help flares as can steroid injections.

At this point you : should see a spine specialist for a detailed evaluation and appropriate testing who can then guide needed treatment as you are not improving on your own.  Testing may range from x-rays, CT &amp;/or MRI with treatment possibly involving PT, injections thru pain management &amp; potentially different classes of medication you may not have already utilized .  If still symptomatic, surgery may be an option .

Pain and panic: In phase ii and iii trials, Pregabalin shows promising anxiolytic action when compared to placebo in generalised anxiety disorder, social phobia and panic disorder. Pregabalin  also may confer some analgesia in neuropathic  pain . The latter treatment with cognitive therapy can only add to the efficacy  to the latter regimen.  Breakthrough panic with Clonazepam as needed sublingual helps.

Spine specialist: In my opinion Grade one retrolisthesis is not a thing ,wiltze classification was for anterolisthesis originally i dont recall him  describing grades for retrolisthesis and later "white and panjabi" did not consider this instability in the orthopedic sense of the word . hand weakness deserves a neurologic evaluation hope this helps
