Insurance should pay: Bad penile scarring after circumcision is a medical problem ; should be a covered expense. Suture tracts or tunnels accumulate dead sloughed skin cells ; keratin which appear like tiny "black heads" ; are also worth opening up or removing. Coding should indicate secondary phimosis or miscellaneous penile abnormality ; thus be covered. Good luck.

Healing scars: A scar forms when there is  healing of an injury to the tissues.  Remember that with almost all wounds, healing continues for months and months as we change the stress on the tissues.  A scar forms as a result and may have many different appearances and colors.  Any dark coloration may fade over time but the tissues never are the same and so some scarring will always be there.

Time: The scar will fade with time. Let it take its natural course.
For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

Scar is always...: A scar is always the residual sign of skin/tissue healing after cut/injury. So, if not related with circumcision, id you have a pimple-like "boil" before? If concerned, go and see doc timely. But, if not hurting and with no erosion or size increase, it is benign with no need to intervene but personal finicky cosmetic reason.

Yes, if you look: Yes, scars are permanent. Usually though the scars will blend in and are not noticeable unless you look for them. Also depends on location and how the injury occurred or by whom the wounds were treated that resulted in the scars.

No: Wound healing is dependant upon a lot of factors and so although i can say that your scar should be completely healed in 10 or 15 days you might be a slow healer or diabetic or malnourished or just have an illness that is slowing your healing right now. If it has been more than 15 or 20 days, i would recommend you go see the doctor and find out why it's healing slowly.

Unpredictable...: Any cut to tissue will heal with a visible "scar'; likewise, does cut for incision. But its visibility is unpredictable, depending upon personal body nature and possible wound infection, most and most of times, the healing of circumcision is uneventful and well in 1-2 weeks. But still remember: incision-related scar takes up to 1 year or longer to mature. Ask doc timely.

Can only speculate: The procedure itself is much more involved: general anesthesia, sutures, swelling, itching, recovery etc in the adult. Add the anticipation & remembrance of the process in an adult. The mental residual is quite different than the infant who, when i ask kids, never remembers that anything was done.The physical scar is similar & a reflection of the skill of the surgeon.

Probably not: Could a circumcision scar be hypertrophic? I have never seen or heard of this happening but in medicine we frequently say that anything is possible. I would add that it is highly improbable though and I have asked collegues and none of them have ever heard of or seen it either. I probably doesn't happen.
