Not magic: Subutex is a great tool but not always enough. Some people with high heroin tolerance dont get complete relief. Because subutex only partially stimulates the opiate receptor there is a limit to what it can do. Above 24 mg no good evidence of increase benefit and not recommended. Talk to your doctor. If relapse is concern Methadone maybe more effective. But counseling and meetings help as well.

Drug dependency: No easy cure, need professional help, subutex  is used like Methadone to detoxify , ovoid group drug addition , unlike Methadone dispensed only from detoxifying centers  ( very few available ) subtext could be given from doctors , in addition nalxone added  to prevent dependence as you mentioned, has to see  doctor regularly and follow instructions ,  will over come.

It depends: First, you have to be completely honest with yourself about your use of Xanax (alprazolam).  Was it prescribed by a doctor that knows your history?  Are you taking it as directed?  Are you working towards weaning off?  If no to any of these questions, your medication use is suspect.  Talking may be more helpful in this instance.

Coming off Heroin.: Detoxing from heroin at home is difficult. Your prescribing md will tell you how to use subutex correctly. You have to be off heroin for some time before you use subutex or else you will have major opioid withdrawal symptoms. Other medications like Clonidine can also help.

No: Be careful.  If the switch is made improperly, you can have withdrawal which might bring on premature delivery.  Methadone can be started without this danger.

Well...be careful.: Yes. Naltrexone blocks opioid receptors, so it's a good option for people who are detoxed already. If Naltrexone is used prior to detox, it will lead to immediate withdrawal symptoms. In addition, use of opioids after discontinuing Naltrexone are at risk for serious overdose, most likely due to loss of tolerance. Naltrexone is best for motivated people who have already undergone detoxification.

Yes: but depends on how you ingested the heroin and xanax (alprazolam).  If they were injected, OD occurs within 10-15 minutes, and certainly no more than an hour. But if you took the Xanax (alprazolam) by mouth, and snorted the heroin, the overdose can occur while the Xanax (alprazolam) is being absorbed, which can take several hours to be complete.
