It's ok: Generally, it's fine to eat an apple. 25g of carbs is only about 100 calories, which is very little. If you drink the normal amount of room temperature water recommended for someone your age and size, you'll burn off at least 100 calories. 25g of carbs is probably not enough to stop the ketosis caused by high protein diets.

Carbs: There is no specific requirement for carbohydrate intake, I assume your intention is to shed body fat .  Restrict carbohydrate intake as much as possible By limiting intake of starch such as grains and roots (potatoes).

type 2 diabetes for 2 years. i have been on metformin, i added farxiga for a year.  when i eat carbs, even avoiding white carbs my sugar goes over 200. but very a low carb diet causes sugar crashes. is it possible i should be on insulin?

Need to know how your sugar varies over the course of the day (x many days), body habitus (weight), hemoglobin A1C, renal function and eating habits

Make an appointment with a Diabetologist (or Endocrinologist) to sort this all out

Guilt?: Don't discount guilt as a factor but, alternatively, you may be having acid reflux with esophagitis.  Try not to eat or drink anything within 3 hours of bedtime, elevated the head of  your bed with bricks/blocks, and consider an OTC med like ranitidine or omeprazole.  PS, ibuprofen,which you list, can make your symptoms worse.
