i am going for a lumbar discectomy 3 weeks after having a steroid epidural for back pain.  is this ok or shall i leave the epidural.?

Right hand left hand: 1st i'd make sure both your surgeon and pain physician both know what the other doctor is doing. That always leads to best outcome. An epidural can be done before discotomy however if you're sure your having the surgery i'm not sure an epidural prior to would be of great use.  Call both doctors' offices and let them know the whole plan!

How many ESI?: If you only had one esi, but it worked for a couple of days, you may consider having another injection.  Sometimes a series of injections (up to 3)  is necessary to provide adequate pain relief.  This is an option if you are reluctant to pursue surgery.  Depending on the size of the disc herniation, surgery may ultimately be the best option.  Consult with a neurosurgeon. Good luck!

i had a discectomy for l5 s1 almost 2 years ago. i still have low back pain. the doctor sees no reason for it and now i have tightness in my upper back. what should i do?

About 10% of : Disectomy patients go on to have back pain due to the disc not being able to function normally as a result of it herniating & the surgery to correct that condition.  Over time, this can get better with good core strengthening but some go onto a fusion for relief.  Weight control, regular exercise & avoiding smoking are also important.

Core strengthening: Congratulations on the weight loss- that is certainly a step in the right direction.  Any issues in the spine cause decreased function of the core stabilizers.  Surgery further decreases the function of these muscles.  A good physical therapist can teach you how to functionally strengthen your transverse abdominus- and look at your muscle imbalances to give you an individualized program.

Probably not much: Modern diskectomy only removes the part of the disk that is ruptured (herniated, slipped) and a small amount  of the disk in the area of the rupture. Virtually disks that rupture are already degenerating, so some height loss has already occurred before surgery. If more height is lost, it is due to continued degeneration and aging.
