One treatment...: ...With elimite, (permethrin) if applied properly, will kill all the mites and eggs immediately. However, the itching can persist for a couple of weeks after the treatment because your skin is still reacting to the infestation. Also, i usually recommend repeating the Elimite (permethrin) treatment a few days after the initial application, to kill any mites you might have missed or any eggs that may have hatched.

Depends: Even if all the scabies mites are killed by the premethrin, itching can persist for several weeks. Persistent itching, especially if there are big bumps, sort of like mosquito bites, can be treated with a topical cortisone (steroid) cream. Usually premethrin is applied twice, about a week between applications. It has to be done right, clothes and bedding washed, contacts treated too.

Be patient: The cream should kill the scabies within the first day of treatment. However, the skin disruption is caused by a reaction to the mites burrowing under the skin and leaving its debris in the tunnels. It can take two or more weeks for the skin to settle down and begin healing after initial treatment.If concerned a second treatment after a week could catch any stragglers, but is probably unnecessary.

Yes: The cdc says that 24 hours after application of Elimite (permethrin) it is safe for you to return to work, etc.

Scabicide &amp; dryness: Scabies treatments often contain a high percentage of alcohol which can be very drying to the skin, especially sensitive tissues of the face and genitals.  Hypoallergenic soaps and lotions which are pH balanced will help the skin recover more quickly, and reduce the itching that can last up to two weeks after treatment.

Healing takes time : the itching can continue for weeks after treatment and peeling is normal. Try a calamine lotion and over the counter antihistamines such as Zyrtec or Benadryl (diphenhydramine) for the itching. Also a topical hydrocortisone cream can be helpful for itching. Vaseline can help with peeling.  Remember to repeat the treatment in about 2 weeks and treat your family members as well.

Not if it's scabies.: "scabies" is a term for pediculosis, a skin infestation of tiny insects.  It usually respond to Permethrin but it may take more than one treatment.
If it is eosinophilic fasciitis, then it is a white blood cell problem and should respond to prednisone. Prednisone can make the skin thinner and more difficult to heal ulcers that occur or skin tears that occur.

23 weeks pregnant. just treated for scabies yesterday w/permethrin cream. still very itchy. is it safe to take benadryl (diphenhydramine)? or anything for the itchiness

One treatment option: I am not an obstetrician but i did want to make you aware of one treatment option - acupuncture. It is very helpful for itching & will be safe during pregnancy.  There are a few specific acupuncture points that should not be needled during pregnancy but these are not used for itching tx.  Congratulations & take care.

No itch your done: A good coat of permethrin should do the trick.  It will take some time for the nodule swelling to go away.   If the itching is gone you can feel comfortable that you did the job.  Remember to wash the bedding, clothes etc and you may need to treat other members of the family.
