Nicotine addiction: Nicotine is a powerful vasoconstrictor. For each cigarette smoked the arteries constrict for about an hour with the result of 50% reduction in blood flow. Nicotine shuts down your circulation. Many with nicotine addiction like its calming effect, which long term leads to ischemic complications like stroke, heart attack, limb loss and sudden death. Fair question: but one that should be directed more towards the people who write the laws, not to a physician.  Alcohol prohibition was tried between 1919-1933 and ultimately repealed with a constitutional amendment rescinding prohibition. Prohibitions against drugs appear sometimes to have very little to do with the actual danger of the drug, and more to do with public perceptions, politics and God knows what.

Its not a contest: What's better for your health, great sleep, excellent nutrition, or a positive outlook on life? Answer: all of the above!  (i don't mean to be snarky). There is no level of tobacco or heroin that's ok. Alcohol is a different story, but it's not clear whether there is a 'healthy' amount, or if drinking a small amount does not worsen your health risk. Heroin and tobacco are both highly addictive.

Gout causes : Gout can be related to diet but sometimes no specific dietary influences are found. It's often due to a metabolic defect whereby you produce too much uric acid. This can then crystalize in a joint and cause severe pain. Have you had your uric acids level checked ? If it's raised and you are having recurrent attacks you might want to consider a medication called allopurinol.

Yes: Gout can be caused by elevated uric acid which can precipitate anywhere in the body, but most commonly in the big toe. Alcohol raises uric acid levels, so in general if you have gout, try not to drink alcohol.

Blood, urine, Saliva: There are tests using any of the above body fluids.  If tobacco and alcohol were last used a month ago they will not show in a body fluid test.  If you were a heavy marijuana user, it is possible that it could still show positive in body fluid test.
There are also drug tests that use hair.  On that test, any substance use in the past 90 days will show up.
