i have a 2 week old daughter. there is a lump on the left side, just above her vagina. it seems to be causing discomfort, as she cried when i changed her nappy. im not sure what it is and if i should take her to the doctor?

I would: This may represent a displaced ovary that traveled down the wrong path (hernia) or it could also be a simple lymph node or growth. If an ovary it would eventually need to be re-positioned, if a growth or node it may need other work. Kids can cry for diaper changes just because, so I wouldn't assume it causes pain.

At best jointly: This depends on the addiction doctor you are working with.  Some are willing to utilize Suboxone and some aren't.  Some doctors will go over the various types of medications for opioid depedence like Suboxone, methadone, etc and work with you to find the optimal medicines based on their risks and benefits.  Subutex is best used to initiate an opioid dependence regimen rather than a long term med.

Meds or not.: 1. Take a history
2. Do a good physical exam.
3. Maybe order lab tests based on 1 and 2. 
4. Make a diagnosis of what is making you ill.
5. Review if this can be helped by diet-such as high blood pressure. Ask you if you can follow a diet.
6. Review the evidence that taking a pill for what you have is healthier than not taking a pill if diet does not work.
7. Discuss if you want to take pills.

Depends: When it could go either way, i describe the surgery to the patient, ask them to realistically assess their own pain tolerence, and let them choose.  Most of the time certain procedures are best done under one or the other.

Soft skin = small: The more delicate the skin, the finer the thread and therefore the finer the needle that is required. The needles/thread range from 1 to 6 (largest to smallest). Generally for the face we use a 5 or 6, for the arms and hands a 4 and for the legs a 3 or sometimes a 2. Size 1 is for heavy duty jobs like holding a piece of surgical tubing in place.

Unsafe: We are in the midst of a coronavirus pandemic. Travel and staying in hotels should be avoided unless in an absolute emergency. This situation should not be different by Easter so rather stay at home.

Always wanted to be : I have always wanted to be a doctor and in particular, a pediatrician, which is what i am. I love helping people, especially children. I enjoyed the sciences in school. I also had a great mentor, my own pediatrician. Pediatrics has allowed me to use my passion for people to help them. Watching my patients grow up and now taking care of their children is so rewarding.

Difficult to know: The risk of transmission is higher indoors than out.Depends also on where all these people have been.If in their own "bubble" the risk may be lower,but the risk is exponential if not(you will effectively be exposed to everyone that they have been with in the past 14days).How many family units does this involve?Only you can decide keeping those things in mind,as well as your own underlying health.

Not yet: Retinal transplantation is a hoped for technique in the future .  It is not possible with current technology.  Stay tuned.
