Sometimes it is a central tremor presenting as head bobbing.  "essential"  means the cause is unknown.  There are various medications that can help reduce the tremor.

Essential Tremor: Primidone (mysoline) is a structural analogue of phenobarbital. The drug reduces tremor amplitude by 60% to 70%. Although Primidone therapy reduces hand tremor, tremors of the head, voice, and other areas improve less. The starting dose of Primidone should be low (12.5-25 mg), taken at bedtime, and gradually increased until tremor reduction is achieved. The maximum dosage is 750 mg/day divide tid.

Variable: Not carved in stone, as we are assessing who would be the best candidate as time proceeds.  My approach would include patients who seem not to respond well to current meds, whose tremors are so intense to be disabling, and those folks who just do not tolerate the adverse effects of their meds.

Yes, not same target: Dbs is most commonly used for parkinson disease, essential tremor, and dystonia. Usually the dystonia is a generalized dystonia although it can be used for more focal dystonia ie cervical (neck) as well.  The target for essential tremor is usually the thalamus, while the target for dystonia is the globus pallidus interna.  If someone was having it for both disease states, a careful plan is needed.
