Mostly accurate: Ultrasounds at that point in your pregnancy is generally accurate.  Remember, however, that the presence of a penis to diagnose a male is generally easier than the lack of one to diagnose a female.  As one of my teachers once observed, "some boys are shy".

our gyn said that our fetus gender is boy 18 weeks after preg. using his clinic ultrasound. he said 100% as baby cooperated and legs open. is it 100%?

While nothing: Is 100%, if the u/s saw a penis and scrotum, only  a very masculinized  vulva could mimic.  It sounds like he/she OB is certain that your fetus has male gender.  You will know for sure after birth. What is the rush? Curiosity and satisfaction have a price, and this isn't worth it.

Good question: I am not sure what the ultrasound technician may have meant if he/she really stated the surviving fetus has penetrated the empty sac of the vanishing twin. If he/she is implying that the membranes of the vanishing twin are somehow wrapping around the surviving fetus, then this is a situation that you would want to have clarified by a Maternal Fetal Medicine specialist. Discuss a referral.

how long does it take for a ultrasound technician to find  a fetal heartbeat in 6 week 5 day fetus on transvaginal us. i asked if she seen a heartbeat & she said no, but she was rushy & moved the wand around in me super fast. ?

Ultrasound: should see the heart beating. If there is an error in the due date calculation, because of late ovulation, this can be what is seen.  Repeat testing of the pregnancy hormone and ultrasound can be appropriate with your doctor's advice.

Poor resolution: Your profile picture is too small to tell anything.  The earliest that you can reliably tell the gender is about 18 weeks.  You cannot tell the gender at 12 weeks gestation.

NO not in my office: Either your dates are off or there is something wrong with the pregnancy, at 13 weeks the margin of error is 5-6 days at the most either way.
