Sounds like: Severe vitamin b1 deficiency. This vitamin has an important part in picking up oxygen from the lung and delivering it to the tissues. Ask for a venous oxygen level and if it is high, it means that oxidation (use of oxygen by the tissues) is also poor and is characteristic of b1 def.

Low: The overwhelming majority of urinary tract infections are treated as an out-patient. However, when there is significant fever and evidence that the infection has involved the kidneys themselves then hospitalization may be necessary in order to use IV antibiotics.

what is the likelihood of being hospitalized for a kidney infection in pregnancy?

Several qualifiers: Hospitalization is needed for those conditions which cannot be managed as an outpatient.The prominent factors include: is the patient able to take oral medications & hold down enough fluids to stay hydrated? Is the germ causing the UTI one that can be treated in pregnancy with an oral medication? Is patient free of other complications? If any of these is no, a hospitalization may be needed.

Possibly: It's possible that the infection was incompletely treated. The urine culture and sensitivities should be verified from the first episode and a repeat urine culture can be obtained.

May be: It is not feasible to provide a meaningful opinion without taking additional history, physical examination and may be some tests. It would be prudent to see your doctor and discuss if you may be depressed. Counter-intuitive as it may seem, exercise may help. Start with low intensity exercise such as walking and gradually increase the intensity as tolerated.
Wish you good health! - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

my egfr has been between 54-58 since 2020. normal bun & creatinine. family hx of kidney disease. hospitalized in childhood for repeated kidney infections and my left kidney is about 1/3 normal size. primary care dismissed my concern. should i worry?

: There may have been misunderstanding, your primary care doc did not see enough to worry about. Best would be to have a nephrologist follow your kidney function, stay well hydrated avoid nephrotoxic drugs and/or supplements.

: Significant kidney infection and renal failure are not typical of COVID infection. If you are concerned about renal damage, consult your internist or nephrologist

May Need Prophylacti: In situations like yours, prophylactic(preventive) long term antibiotics may be indicated. You should raise this question with your treating doctor or seek a second opinion from an Internist(Internal medicine specialist).

See urologist: Acute pyelonephritis, or kidney infection, can lead to kidney abscess, which may need additional treatment, including surgery.  If the infection was due to presence of a kidney stone, this can also lead to persistent symptoms.  You should see a urologist for additional testing, including urine culture and urinalysis, possible ultrasound or ct scan.

how can a kidney  infection put you in the hospital?

Acute pyelonephritis: This can be an extremely severe infection which may have been caused by infection elsewhere extending to the kidney, or the primary kidney infection creating sepsis systemically.  The extent of disease, age of the patient, organism and other factors will determine the need for hospitalization.
