If it's periodontal disease, the only "home remedy" available is routine and meticulous brushing and flossing along with an antibacterial mouthwash like listerine or crest prohealth. 1) go to a dentist and pay out of your pocket for what appears to the need to have the tooth removed.
2) check to see if you have a program like mission of mercy in your area.  Usually this is a volunteer operation that may be able to remove you tooth at no direct cost to you.
