It depends: You need to know first what is the cause of the enlarged lymph nodes. There are a lot of things that can make your lymph nodes enlarged. You will need to rule out infections, autoimmune disorders, immunodeficiency syndrome, malignancy etc. The best treatment has to be directed to the etiology/ cause of the problem.
Many times, antibiotic is prescribed if infection is thought to be the cause.

Many possiblities: Lymph nodes enlarge for many reasons, often because they are fighting inflammation, which is one of their primary functions, but they are also susceptible to primary tumors or metastatic malignancies, non-infectious granulomatous disorders like sarcoidosis and others. In order to determine the cause they may have to be biopsied, although a full evaluation of your health is warranted.  Good luck.

: The reference ranges for your serum labs are needed before the numbers can be interpreted. I would speak with the doc who ordered the testing in the first place.

Yes, regional metast: If this is a case of lung Cancer, then lymph node spread to the hilar areas and mediastinum means regional spread of cancer. This is still treatable but patient needs to go see a Cancer specialist(Medical Oncologist)  at a recognised Cancer Center or  Medical center where expert specialists who treat Lung Cancer would be more easily available.

Accepted Answer

Autoimmune: 1:320 is a high titre of ANA, so likely positive for an autoimmune disorder. The diagnosis is made from multiple factors, so that test alone is not diagnostic of any specific disorder. Adding the hilar and mediastinal lymphadenopathy makes Sarcoidosis more likely, though there are many other causes of each finding.  Your PCP may want to send you to a rheumatologist.

YES: It can, but is more likely secondary to the etiology of the hilar adenopathy

I have not: personally seen this but I can attest that is is a RECOGNIZED ,well documented side effect of Dilantin!

my mom has minimal atelectasis no mediastina or hilar lymphadenopathy her symptoms r still same i'm confused she is continously coughing plz help :(

Speak to her MD: I am uncertain how you know about atelectasis and not having mediastinal or hilar lymphadenopathy.  Have you seen her studies?  Hopefully, she does NOT smoke.  
Meds can be given to suppress cough, but usually cough is a protective mechanism.  Please speak to her MD and try to help figure out what is causing her cough and then she can be treated specifically.  Good luck.

what are some possible causes of bulky mediastinal lymphadenopathy(other than cancer) involving subcarinal, bilateral hilar and paratracheal nodes?

Sarcoid: could cause adenopathy in those regions, as well as other granulomatous inflammatory processes.  Infectious processes such as histoplasmosis could do it. Certain chronic exposures such as berylliosis and silicosis could cause mediastinal adenopathy, but usually the history and other findings make those diagnoses more obvious.  Unfortunately, lymphoma is the major consideration in most cases.
