Steatosis: Fatty live is most commonly caused by a condition know as non-alcoholic fatty steatosis or nash. Alcohol consumption may also contribute to fatty liver. Mono can affect the liver but usually resolves with time.  Disseminated herpes zoster which causes shingles can also involve the liver, mainly seen in the immunocompromised state. Follow-up is recommended.

Higher than normal: Alcohol intake is the usual cause for elevated ast, if you drink, stop entirely and re do the test in 3 months. High ast can also be due to damage to muscles, heart and red cells. Get worked up: His physician will probably repeat these with him off alcohol and all meds after two weeks. If they're normal, that's his warning. If not, a workup for hepatitis b, hepatitis c, autoimmune hepatitis, hemochromatosis, wilson's and antitrypsin deficiency as appropriate. If none is found, he may perhaps have recommended an even more fitness-focused lifestyle. Best to you both.

my liver test came back with my alt at 6, and ast 54. the ratio came back very high. i dont drink, perfect weight, take no meds, and feel great. should i be concerned?

: The ratio is high because one number is much higher than the other. Sometimes, this can be an inflammatory marker so if these numbers have never been abnormal before, it is worthwhile to repeat these labs in a couple of months. Did you take any over the counter meds or supplements? If the AST remains abnormal, your doctor can refer you to a liver specialist. Good luck!
