Liver or muscle: Alt is a liver enzyme that is also present in muscle tissue. High levels could mean some inflammation of either of those areas but you need other, more specific tests to analyze the alt level correctly.

Find out why: Has your doctor done test to figure out why the alt is high?  If not tests to rule out hepatitis are indicated.  Other problems  such as medicines can lead to this also and last but not least is fatty liver disease.  An ultrasound of the liver can help to see if this is a likely cause.

Liver impairment,,,: Not all these symptoms may fit but the high ast and alt and low Albumin and possibly the low platelet count indicate deterioration of liver function.  You've also got cirrhosis of the liver which practically leads to end stage liver disease in time so it's clear that the liver is impaired.  Your medications have Insulin resistance/type 2diabetes as a side effect which would explain your thirst.

just had 2 iron infusions (anemia due to very heavy periods and ppi use). lab work 1 week after infusions shows a high alt level (42) and ast higher than usual also (27). both are usually in the teens- related to the infusion? i do not drink?

Could be: It could also be an intercurrent viral infection. One reading that is still in or near the normal range is not a cause for concern. You may wish to talk to the doctor who ordered the tests.
Your doctor can prescribe oral contraceptive that can limit your period to once a year.

Viral hepatitis?: Both hepatitis b and hepatitis c can be causes. Excessive alcohol consumption can also do this. "fatty liver" is often associated with metabolic syndrome and elevated lipids, but some people can have lower triglycerides. There are a number of other causes of hepatitis as well. You need to see your doctor for a full evaluation.

Fatty infiltration: Viral and autoimmune hepatitis always needs to be considered and ruled out. Asymptomatic fatty infiltration can cause ast, alt elevation. That condition is generally not of any clinical significance other than periodic lft should be checked for stability.

Abnormal liver tests: Occasionally stones in the common bile duct cause abdominal pain and abnormal liver function testing. Pancreatitis or intestinal adhesions are other causes of recurrent abdominal pain, but adhesions don't affect the liver as a rule.A rarer cause is porphyria. I assume you have been evaluated by both a gastroenterologist and an abdominal surgeon.

You need a workup: You're a young man and if you exercise and feel well, I'm not concerned about the CK. Around 1 person in 4 has a positive ANA, usually low titer. Your liver tests will be repeated after 2 weeks off alcohol / meds if applicable; if still up, you'll be screened for hepatitis B and C, Wilson's, hemochromatosis and autoimmune hepatitis. Your body may simply be asking for exercise &amp; better eating.
