Head lesions: spontaneous lesions on a child that are causing pain is a concern. please take your child to a pediatrician to get this evaluated. to help you the most a physician needs to see the actual lesion and if needed perform a biopsy.

Skin lump: Usually possibilities include a cyst, an abscess, a lymph node, or a lipoma. However there are many other rarer possibilities.  It is prudent to ask your doctor to evaluate the lesion.  Especially because it hurts to touch it. That suggests that it could be inflamed or infected.

Mastoid process?: The normal skull has a hard spike like projection located behind &amp; at the level of the middle of the ear called the mastoid process. Multiple muscles dealing with the jaw attach to this projection. Other possibilities include lymph nodes located under the skin around the ear that vary in size from that of a pea to peanut. These are part of the bodies germ fighting crew &amp; may enlarge when they are.

Get it checked: This is something you don't want to mess around with.  Even though you are only 20, you need to be sure it isn't cancer.  Perhaps you were injured - do you remember getting hit in that area?  Nevertheless, go see a doctor.

Exam needed: This could be an ingrown hair, inclusion cyst,wen or benign fatty tumor(lipoma). A lymph node in that region is less likely. I doubt serious pathology, but a physician or dermatologist would have to examine you for diagnosis and advice. The tenderness could be simple irritation or infection, and that is why you should check it out. In the meantime apply warm compresses.

Lump: without exam would be hard to figure out.  You should see your physician to have it examined.  Could be lymph node but without seeing it and doing an exam would be impossible to diagnose.

Either or see a doc: it could be either a lymph node and that will explain the tenderness  and mass effect
it could be an oil gland cyst what is called sebaceous cyst that could get hard like a solid mass and it could get infected and turn to abscess like structure
either way you need to see your doctor for evaluation and proper treatment at least with antibiotic before the infection spread
