Trigeminal Neuralgia: If by tn you mean trigmenial neuralgia then the diagnosis can be confirmed with physical exam, reflex testing, blood work, and/or an mri.

Trigeminal neuralgia: The presumed cause of tn is a blood vessel pressing on the trigeminal nerve as it exists the brain stem.  The  compression causes the wearing away of the protective coating around the nerve(myelin sheath). Other causes include nerve damage, genetic tendency, stress is an aggravating factor. This deterioration causes the verve to send abnormal signals to the brain.

Lucky it's not: Good question. I don't think there is a simple answer for this. Possibly the irritated trigeminal nerve fatigues or is suppressed by the brain. A similar situation might be epilepsy where the damaged area only gets triggered periodically. Most people with tn couldn't stand having the pain constantly.

Intermittent: Trigeminal neuralgia pain occurs when a branch of the trigeminal nerve becomes inflamed or over-excited. When this is relieved the pain resolves, but can be triggered again if the nerve is irritated.
