Serious consequence.: Depending on the size this could have very serious consequences including permanent brin damage and death. You need to listen to the advice of the neurologist and neuro surgeon. They spent most of their young lives learning how to treat these life threatening conditions.

fell from a horse 2 wks ago & told had grade 3 spleen lac w/ lrg hematoma. surgery found to be a massive cyst and 3x larger then normal spleen. fall ruptured cyst. what causes these 2 conditions at the same time? no prior s/s or health issues.

Cannot say in your p: particular case and complications, serious complications can cause this. If the flow of fluid around the brain is clogged from blood, increased pressure in the head can do this. Sometimes the pressure is not elevated when measured BUT increased space around the brain implies the fluid is increasing (normal pressure hydrocephalus). causes dementia if not shunted away from brain

Generally surgery is recommended if the hematoma is causing symptoms or is large on ct or mri. The primary risks of surgery are infection and rebleeding, possibly requiring reoperation. The risk of brain injury or long-term side effects is low.

Can be normal?: You haven't really given much information here.  Minor hematomas can be normal and managed with heat, compression and observation.  Larger ones may need draining.  I strongly recommend you se your surgeon sooner rather than later to avoid unwanted complications!
