Possibly: The injection could have caused a small leak of spinal fluid and can be giving you a spinal headache.  You should see your pain mgmt doc asap as a simple blood patch could reverse this easily.

Headaches . . .: New onset headache in someone who never has headaches is an alarm bell for us regardless of age, so best to get evaluated sooner than later. Unfortunately, I'm answering your question about 9hrs after you posted, so I hope that she's better whether you took her to ER or waited. I would recommend an emergent evaluation especially given recent history of contaminated steroids a few yrs ago.

cortisone: Most injections of cortisone begin working within 24 to 48 hours, but can take as long as a week.  If you don't notice relief within a week...Then it probably wont work.

80-90 %: In my experience if you have a definite diagnosis of impingement syndrome(made by history, physical exam and confirmed by a good mri)your chances of improvement after a lidocaine/marcaine/steroid injection that is in your subacromial space(best done by ors) is 80-90%. In fact, if someone w/'impingement'does not get better after injection than the diagnosis may be incorrect or misplaced injection.

in your practice experience how effective is a guided cortisone injection for frozen shoulder if caught early, say two months into the freezing stage?

Possible help: Glenohumeral injections or steroid are an acceptable practice in conjunction with aggressive physical therapy in an effort to improve range of motion in 'frozen shoulder'.  Still, the average duration of symptoms can be betwen 6-9 months and sometimes longer.
