An appendix of a testicle (a small lump near a testicle) might be a lump on the testicle. No lump growth or change in many, many years makes a lump likely non-cancerous... but new lumps are unknowns and need to be examined closely.

To verify and/or get a definitive diagnosis, best to see a urologist when lump occurs again.

Must rule out cancer: A swollen testicle or one with a lump, especially one that feels firm, is a sign to go see the doctor this week. One can see his primary care doctor first, who may refer to the radiologist and urologist to further evaluate and do an ultrasound scan, if testicular cancer is suspected. If there is a cancer, it is treatable by removal of the testicle, and getting follow-up care.

: I would feel that a very firm mass within a testicle wouldn't come and go and that includes spermatoceles. You're correct in going to a physician for examination,probably will be scheduled for an ultrasound and/or referred to a urologist depending upon the findings.

See below: There are a number of causes of “lumps” in the scrotum and all scrotal masses need to be examined by a doctor, even if not associated with pain or other symptoms. If the “lump” is arising from testicle, the most important cause to rule out is Testicular Cancer. If separate from the testicle, an epididymal cyst or spermatocele is the most common. Other common causes: epididymitis and varicocele.

Here are some ...: Is a "tiny" lump on scrotal skin or its wall or inside the scrotum? Being on or in scrotal wall, it can be easily palpated by ordinary hands; if inside the sac, a tiny lump is not easy for ordinary inexperienced hand to differentiate if it's on or inside or outside the testis, but such indeed terrifies some men. There's a small structure up there called the appendix testis (yes, appendix testis) that can get clogged, which doesn't mean much but should be verified just to make sure it isn't something more serious, especially at your age. It hasn't been emphasized enough that men in their teens and twenties check their testes for masses. I'd suggest getting your girlfriend to do it...Fun
