Tense?: This is usually a reaction to stress.  Stress reduction techniques can be useful.  There are many books on this subject if you wish to pursue it - consider yoga, self-hypnosis, transcendental meditation, and behavior modification techniques.  Brief counseling can also be useful as this is a form of obsessive-compulsive disorder and amenable to psychotherapy.

how can i tell the difference between chest muscle pain or heart pain? they are so close together i am having a hard time distinguishing them apart.

Distinguishing Pain: Muscle related chest pain is  sharp reproducible either by pressure or by arm movements or applying load to concerned muscle ( tightening or contracting muscle. Heart related pain . Is usually vague achiness and usually worsens with exertional activity. There is usually feeling of breathlessness with exertion.  Hope that helps. In most chest pain cases it is best to see a physician  right away.

Need to see doctor: ASAP! Your cause can be cardiac, anxiety or thyroid related and without a physical exam done, it's impossible to tell what's going on. So you need to call your doctor, and see her/him quickly. It can be all 3. At your young age, anxiety tops the list, but stomach discomfort is another indicator. So, don't delay and get help!

Fibromyalgia ?: 18y male has ongoing "muscle spasms" &amp; mentions head, groin &amp; chest. If lab tests are normal consider Fibromyalgia &amp; because so many different body regions &amp; tissues are involved. See a Rheumatologist for an opinion. In males, Fibromyalgia seems to follow upon trauma &amp; mechanical injuries to low back &amp; pelvis; especial after extreme sport injures such as skateboarding/snowboarding.

Unusual location: The lower left back is an unusual location to have pain from a myocardial infarction (heart attack). However, that doesn't mean that something is not wrong - pain is usually a warning sign. If you've not had any trauma, then I suppose muscle strains or back injury is out the question. The colon (colitis) or kidneys (ureter infection/stone) or even AAA (aneurysm) can cause it. Talk to your doctor.

Pain : You are probably correct about musculoskeletal but if you have pulmonary embolus history should discuss with your doc.
