Live or dead cells: All acne lesions begin as groups of abnormal cells, which because there are extra cells will feel firm. If the process damages the tissue enough to cause it to liquefy (which allows the white cells to chase the bacteria better), it turns soft. Best wishes.

What have you tried?: I'm going to assume that you have tried topical benzoyl peroxide 5% 2x/day for several weeks and you use folk remedies only as adjuncts if at all. Half of acne patients need prescription-strength rx. If topical clindamycin, the oral contraceptive pill (for ladies), oral tetracycline, and topical spironolactone have failed, consider Isotretinoin from a dermatologist. It's rough but it works.

: Difficult issue.  I would invest a few lbs and subscribe to uptodate.com for a month and read all about these issues. Then you will be better prepared to discuss options available in UK with a Dermatologist over there who will be better able to suggest what to do in women like her.

Acne: This may be all about your hormones and genes and the skin type you have.  If you are doing everything right and still having trouble you may need some prescription medication to supplement what you are doing naturally. It is not impossible to get improvement.  You just may need help.

See a dermatologist for evaluation & suggested treatments.  In the interim, do not pick or manipulate the bump.

First treatment is focused on drying the skin with Benzyl Peroxide.
Common treatments are  topical or oral antibiotics such as tetracycline or clindamycin.
The age of a patient and gender also will dictate what medicine you should be on. 
If you  have Cystic Acne. , you  might need to on Acutane  treated only by dermatologist., since it can  have side effects
