Thre are several: Possible reasons. Trauma, sleep positon, infection, tendonitis, especially given age. Radial sided tendonitis.  May be a common issue such as de quervains, one may need to refine the comlaint, ie , is the swelling in the whole hand or just feels swollen. Is the tingling ing extend up the arm or just into the hand and forearm?  Is there neck comoression ad the tingling hand "feels" swollen , .

There are multiple reasons why you are having these symptoms. It could be anything from an infection, joint inflammation, blood clot, among others.

Swelling: If there is associated redness there would be concern for cellulitis which is a serious skin infection. An allergic reaction would be another possibility. A blockage in a vein is possible but the swelling would persist with that. Having your physician examine you would be best.

Bruised: Bleeding occurred at the injured site and blood has leaked down toward the hand because of gravity. I'm assuming you didn't injure the hand also. The discoloration is from breakdown products of blood and will take days to clear. Elevate your hand to reduce swelling. However don't hesitate to get your hand checked by a physician if swelling worsens or hand pain is present.

Benadryl (diphenhydramine) and time: Benadryl (diphenhydramine) should help, that and a little time. If he gets numbness or loss of sensation because of the swelling then you should see a doctor right away. That is not very likely.

Ask surgeon: to be sure, but often a light compression sleeve will help reduce swelling after this type of surgery. The reason it is important to ask your surgeon first however is because of the ulnar decompression part. If you had a great deal of scar tissue or other anatomic abnormalities, you want to make sure they are not worried about compressing the ulnar with compression sleeves. Otherwise should help.

See Ortho: I would visit the orthopedic surgeon who performed the surgery. 3 and 1/2 months is usually adequate to heal enough to be pain-free. Is it possible that you may have reinjured it somehow?

Yes: That is a very tender area of the body and those particular procedures can cause lingering pain. It will improve but be sure to tell your surgeon and ask for a pain consult to find out your options.

It takes time: By the information you have given us, it looks as though you had either long standing ulnar nerve entrapment or a severe case of one with significant muscle wasting. If that is the case, decompression of the nerve does not reverse the condition rapidly. Regain of muscle strength and muscle mass takes weeks to months. Numbness is even longer and sometimes is permanent.
