I doubt if the current pain has anything to do with the fall years ago unless this pain has persisted in the last 4 years.

You need an xray...: You may have fractured a small bone in the wrist, near the thumb, called the scaphoid.

It's common for fractures of that bone to be missed (because they,  eventually,  stop hurting), but left untreated, can cause MAJOR problems in the future.

You shouldn't: Hand pain in a hand containing an IV could indicate infiltration of the iv. It is a relative indication to remove the IV and restart elsewhere if it is needed.

Breaks or fatter pen: If you hand hurts from writing for long periods of time, try to take breaks and stretch your fingers back.  A fatter/wider pen may help if you have certain types of hand arthritis.  If you're not sure of the cause, check in with your local hand surgeon.
