What type of tests?: A lot could be going on, but what level of diagnostic testing has been done? Have you gone to specialists or to one of the oral medicine experts at the dental schools close to you or the hospital dental divisions where you live. You may have a multi- disciplinary issue. Fistula....: I personally advise you first see a dentist to discuss your options based upon a clinical exam and x-rays. You should understand the pros and cons of each option and prognosis of Endodontic treatment followed by restorative treatment. Your General Dentist may then elect to treat you or refer you to an Endodontist (or Oral Surgeon).

Since you are in pain, especially with an ongoing problem that has not gone away for years, the logical and smart approach would be to see a dentist to evaluate the need to remove your problematic wisdom tooth and make sure you have no other dental problems that require  treatment.

See an ENT: Ent specialists (ear nose & throat) would be a great place to start.  One the ENT resolves the sinusitis, if the pain persists you can go to see a dentist.

Fracture: Your tooth may be fractures, you may have gum disease, or you may be grinding. See a local dentist for an exam to find out for sure.

Unlikely: Sinus infections or inflammation do not generally cause pain on lower teeth.  Gum recession does not cause spontaneous pain, but instead may cause pain or sensitivity when eating, drinking or brushing.  You need to see a dentist as soon as possible to determine the cause of your pain and render appropriate treatment.

Anesthetic: One of many local anesthetics is chosen, depending on your need, spread of infection, etc.  You can't do this yourself at home.  If you have tooth pain, you have a badly infected or damaged tooth.  Even if numbed, the infection will spread into the surrounding tissues and your blood stream.  You need to have the infection treated.  Please call your dentist now.
