The neck/cervical: spine joints, like all joints, are bathed in an oily-like substance called synovial fluid which facilitates smooth movement. Theories about crepitus (these noises) include changes in synovial joint fluid pressure or gaseous bubbles forming or bursting. Or sounds from ligaments or tendons as they move over bones, etc.  Bone on bone grinding due to worn down cartilage from arthritis very rare at 23.

when i am stressed and grind all night, i can barely move my neck from side to side the next day. why is that?

Tight muscles: Spasms in overused muscles will cause pain, decreased movement.  Learn ways to deal with stress, de-stress whatever you can, consider getting a mouth guard from your dentist, or ENT doc to minimize the effects of grinding.

i've had pain in the top of my shoulders and in my neck for 17 months now and i can here grinding noise and i feel like i'm swaying all falling over

Meniere's: Recommend consultation with otolaryngologist (ENT). This may represent Meniere's disease. Neck stiffnes and pain are commonly seen in the setting of Meniere's and that would explain your vestibular dysfunction (dizzy feeling) Could be cervical spinal stenosis depending on your history. Could be a double crush meaning symptoms are unrelated and caused by two separate things too.

Muscle: It is likely muscle bands / spasms from tension or injury or overuse. They rub over one another when rolling neck/shoulders. Can also be local fluid build up (same causes). Often improves w/massage and try cold compresses to area. If not improving, dr could rx med to help reduce spasm.

Probably not: The medical community used to think that cracking the knuckles will lead to arthritis etc...Currently there is no concrete evidence to support that theory. So, cracking your knuckles, joints, back, neck without pain...I am not concerned.
If you are worried, check with your doc..

See you dentist: This could be early signs of TMJ which is the joint between your jaw and your skull, or just grinding which you may need to use a mouth gaurd.

Grinding - tissue: This suggests that  something is rubbing: bone on bone or on other tissue, tendon rub, cartilage rub. When you see you physician tell him of her about this and also if there  is pain or other symptoms there. You need  to pay close  attention as to where it is coming  from.

Neck pain: There may be inflammation around the nerves coming out of your spinal column in your cervical (neck) region.  There may even be a malalignment of the vertebrae.  Muscle relaxers, anti-inflammatories and other medications can help on a short term.  You can see a WELL QUALIFIED chiropractor (but be careful, it's your neck!).  Pain management and Orthopedic Spine doctors are specialists for this.
