Thin cartilage: Chondromalacia is thinning or loss of cartilage from multiple causes, mostly trauma from direct injury or overuse. Rest &knee joint support, including upper leg muscle strengthening are the main rxs. Adjunctive relief may be possible from a 7 active ingredient glucosamine product and epa omega 3 fish oil at a dose of at least 600 mg, but a response should not exceed 4 weeks. Wt control is key!

Usually time: True chondromalacia is caused by irritation of the back of the kneecap against other structures of the knee. Usually pain is a main issue. If all you have is cracking, this does need need specific treatment. Surgery is not indicated for this problem altho may help some conditions that are similar. Quad strengthening is key, and tylenol/otc meds often helpful. Ice after exertion, too.

If PT with exercise and icing/taping did not help,an Orthopaedic Surgeon can evaluate you for patella alta or baja, put a camera into your knee to look for a plica, remove damaged cartilage, and give an expert opinion of your biomechanical foot/knee/patellar alignment.

Try conservative rx: Seems like most of the damage is under the patella. If you have locking you might need arthroscopy, I would first try terminal extension excercises training the quads without putting much pressure on the patella. In addition research has shown that Asperin could help the surface cartilage heal. In addition, MSM (Puritan Pride) could be helpful as a nutraceutical supplement.

In some cases x-rays and/or  MRI are used to evaluate the degree of degeneration.
