Shoulder: It depends on severity of sprain and what was injured. Usually you stop using sling when there's no pain but should be combined with PT to preserve range of motion and avoid stiffness.

Probably not: I would not expect a shoulder sprain to increase your creatinine.  Some supplements can increase your creatinine.  
Other muscle injuries can indirectly increase your creatinine.

Let me try to help: Sprain means the muscle or the ligaments are exposed to stress beyond capacity so they stretch or tear. Sprain usually put in grade 1 or 2 or 3. Three is the worst. Good luck.

Hospital emergency: rooms are prepared to treat all shoulder injuries including fractures, sprains and tendonitis.  Clinicians can help set shoulders if displaced, diagnose fractures and cast or sling, give strong medications to alleviate pain, and help set you up with specialists that you can follow up with.  Without such services, you will not be able to get the help you need.  Some primary care offices can do so.

Sprain: I'm assuming you've been diagnosed with a back and shoulder sprain, meaning that you've had a stretch/pulling/twist injury do those areas. These type injuries almost always resolve with time and therapy and NSAIDs.

Spine consult: You need a referral to a spine surgeon for X-rays of your spine to rule out vertebral fracture common in your age group and also a bone spur that can irritate the spinal nerve running from your spine to your bicep=radiculopathy. SSRIs and gabapentin good for this pain but if you have weakness surgery may be indicated for a herniated disc or compressed nerve. Stay calm and see spine surgeon.

Inability : Inability to move your arm is a serious issue. The three main issues include a tendon tear, a shoulder dislocation or an injury to the axillary nerve. All of them require further orthopedic management. The strongest possibility of the three is a rotator cuff tear. Seek care as soon as possible.
