Probably contusion: A direct blow to the knee most commonly causes a contusion, a bruise to the soft tissues and the bone.
It can also cause bursitis, an inflammation of the tissue in front of the knee.
If there is significant swelling or instability, it may be worth seeing a doctor to have it examined to be sure there is no internal injury.

Many things: This could be many things. You really need to be evaluated by an  orthopaedic surgeon.
It could be a muscle strain, a capsule strain, a meniscus tear, arthritis, and the list goes on.  You would benefit from an exam. Good luck.

Unlikely: These are all similar treatments.  If you failed one the others are unlikely to work.  4 injections of Hyalgan is an unusual number.  Did you complete the recommended treatment?
