Can be normal: Healthy persons can have normal juvenile large platelets. The more juvenile, the more larger platelets there are. EDTA in testing of MCV can cause clumping of platelets which may make them appear larger than normal. However, abnormally large platelets may be seen in Glanzman thrombasthenia, primary myelofibrosis or in myeloproliferative disorders. Discuss findings with your DR.

Both are normal.: Most labs use as a normal range for platelets to be 150, 000 to 450, 000.  The slight increase you wrote of are not concerning.  Anisocytosis refers to change in size---not all the same size platelets.  Large platelets can be associated with many things, including bone marrow replacement with younger platelets.  Smaller platelets can be associated with different genetic conditions or may be normal.

Blood smear?: That gives information about the number and shape of blood cells. Tear drop may indicate myelofibrosis, severe iron deficiency, thalassemia major. Schistocyte or helmet cell seen in hemolitic anemia like hemolitic uremic syndrome and artificial heart valves and some other microangiopathic diseases. See your physician for clinical correlation of the results. Shapes are not diagnostic of a disease.

Not the cut: Follow it. If you are iron deficient, find the cause and get treated. You may have a check to see if a particular mutation is driving it. This is serious but probably not catastrophic.

Depends: If hemoglobin level is normal, it does not mean anything. Some hemoglobin variants produce oval rather than perfectly round red cells and then it depends on the type of hemoglobin variant. A few ovalocytes are of no consequence.

: A peripheral blood smear may show circulating leukemia cells but not other cancers as a rule. In PV, a smear may show crowded platelets&amp; white blood cells, but the diagnosis requires certain blood count levels  and bone marrow findings.

when looking at a blood smear of a patient with acute lymphoblastic leukaemia, how do we explain what we're seeing and what's going on?

: When looking at a blood smear under the microscope, the doctor is able to identify the abnormal cells. There would be cells called blast cells for this diagnosis and these cells can be identified as well as quantified-how much of the cells are these abnormal cells? Luckily, ALL is very treatable.
