Many things: Urinary tract infection can cause fever, abdominal pain and frequent urination.  Nose bleeds in children is relatively common and usually due to allergies and excessive dryness.  A comprehensive exam and lab tests would be required to determine the cause of these symptoms and how many of them are linked.

Interstitial cystitis reacts to urinary acidification from beverages or food like fruit juice, tomatoes or irritants like caffeine. Milk or making urine alkaline with sodium bicarbonate can help &amp; try bland diet with boiled or baked foo &amp; avoid fried or spicy food. Symptoms most often from UTI, but, can also be from stress or anxiety.

Some foam in the urine is entirely normal. Protein leaking from a leaky kidney can cause kidney FAILURE if not treated. The one patient I have with NULL DISEASE, gets this every 2 years, gets treated for 1 month and is OK.
Diabetes, severe hypertension, other kidney trouble, moonshine poisoning, several other unlikely possibilities.

Many things: affect frequency of urination, including amount and frequency of water intake, physical activity, and even the weather (the kidneys will adjust output if it is hot and you sweat more). Of course, some diseases, such as bladder infections, can increase urination, but usually would be associated with pain and burning with urination. If he develops these, see your doctor.
