Testosterone: There is no "accurate" or fixed dose.  400 mg/wk is a big dose. Too much emphasis is placed on blood levels. If you are hypogonadal, treatment should be given to achieve objectives, eg growth, puberty, male characteristics, bone health, etc.  More T is not necessarily better; a blood level of 1000 is not nec better than 500 (just more side effects).  You should be seeing an Endocrinologist.

If you used to get great erections and were horny most of the time, and it's largely stopped, you don't have XXY or primary hypogonadism. Losing libido at your age is often due to prolactinoma and/or hemochromatosis with normal testosterone levels.

Excellent: At age 19, I am surprised you had a Testo level...but it is fine.  If there are other problems then those would need to be considered

i had done a free testosterone test my levels are 28 pg/ml report shows the normal range is 1 to 28 pg/ml while for women report shows normal range less than less than 2.85 i don't understand how can a level of 1 pg/ml be normal for a man?

: I dont know of the lab normals in India but here the levels in males is 50-200pg/ml and females 0-4.2. Testosterone levels vary at times of day- highest in a.m. and lower towards the p.m. hence the variation. Have your ordering doc explain the results of your lab.

i am 38 . my total  testosterone is 291. and my free testosterone is 5.9 . normal lab ranges are 241 - 827 @ 5.1 - 41.5 ... i am symptomatic.  how does my urologist goin to look at those values. what they indicate for my age ?  thanks.

Low-Normal. : In and of themselves, they’re in a low-normal range and as such they’re technically normal. However, if you remain symptomatic it would be advisable to bring your levels up to the mid-range.

: There is no way to answer this question with certainty.

There are a number of factors that influence penis size, including genetics, hormones, among others. FSH, LH, and testosterone vary throughout a man's life. There is no way to know what those hormone levels were years ago. Even if the levels were known, it's nearly impossible to accurately determine the degree of impact on penis size.

my free testosterone results: 6.6 on a scale of 0.1-6.4. little high. i have been trying to conceive with no luck. my doc. prescribed metformin. i dont have pre-diabetes. can the .2 difference cause this much issue? everything else is normal range?

If the levels are low your doctor will look for the cause and plan management or referral to an Endocrinologist.
