Infection: An open sore on the foot of a diabetic is serious. First, diabetics have more difficulty fighting infection. Next, poorly controlled diabetics can have decreased blood flow to their extremities. Lastly, they can experience poor wound healing. All of these together can create a situation that will require a very aggresive approach to prevent spread to the rest of the foot.

Get seen: This could be serious, any of several of the blistering skin diseases. If you are taking any medications you don't really need, discontinue them.

Dry thoroughly after bathing. Powder in shoes can also be helpful. Additionally, topical anti-fungal creams, solutions or sprays may be needed. If a prolonged problem, see a podiatrist.

I.C.E.: A sprain is when a ligament or tendon is injured by overstretching.  There may be minimal or quite a bit of damage to it.  Inflammation (redness, warmth, tenderness, and swelling) occurs as a natural healing process.  The key is to minimize further damage while keeping the area flexible and mobile.  Ice massage (24-36 hours), compression and elevation are the keys to maximize healing.

On awakening?: Pain on the bottom of the foot, and bottom of the heel, worse on first steps is often plantar fasciitis (inflammation of the thick lining on the bottom of the foot): rx'd with stretching, anti-inflammatories.  Other problems could be fractures, neuromas (painful nerve swellings), less likely referred pain from sciatica (infammation of nerves in back).  See your doc.

Avoid excess H2O: Avoid letting your feet by wet/sweaty for too long a period of time. Use a mild abrasive product like "ped egg" to remove excess dry skin. Apply urea-containing creams such a Lac-Hydrin or a product such as udder balm daily.
