Report: Essentially, this says that cysts seen on a previous study are not seen now.  However, in the left tubal complex, there is fluid which is not in a single balloon-like pocket, but rather where there are bands that break up the inside of the balloon into a number of parts.

last month i had free fluid in the adnexa region aspirated. this month during the follow-up visit the fluid is back and even larger than before. there is no cyst. what could be causing this? note: i had endometriosis surgery almost two years ago?

: A spontaneous rupture of an ovarian cyst (or two) can cause free fluid in the area behind (posterior to) the uterus.  Ask your GYN physician for guidance.

Sounds like a cyst: Without seeing the entire report and images we can not interpret the results. It sounds like they are describing a very small 2 centimeter ovarian cyst which is typically a benign finding. A trace amount of free fluid is generally a normal finding. Contact the ordering physician who can interpret it in the proper clinical context

Early PG ultrasound: This is typical of an early pregnancy ultrasound as a gestational sac can barely be seen if at all. The corpus luteum is a "yellow body" within the ovary that produces the hormone progesterone and sustains the pregnancy until the placenta develops after 12 weeks. Free fluid is just that, a small amount of fluid inside your abdomen that was seen. Endometrium thickness is support for a new pregnancy
