Reconstructive Surg.: "flap surgery" refers to transferring tissue to replace missing form or function.  Depending on the deficit, the flap can contain skin, fat, muscle, and/or bone.  The definition of a flap implies that the blood supply is also being transferred together with the tissue type(s) and flaps are often used to cover difficult to heal wounds.  Flap dissection requires intricate anatomic knowledge.

Activity post surger: You can do most anything 6 weeks after surgery assuming there are no post surgical issues.  Your surgeon will tell you for sure.

Insurance vs private: Unless a dentist is a participating member of an insurance plan and bound by a contract, he\she can charge whatever they wish for any service. Each dentist may set their own fees based upon their knowledge, experience, overhead and personal desires.  Some insurance companies pay according to a schedule and percentage, others just a percentage, but there are generally limits to coverage.

Office policy: This question needs to be directed to the dentist who is performing the procedure. Since each office has a different policy, there is no one answer. You are asking for a 100% guarantee... Not possible in any medical or dental procedure. You need to at least assume some of the financial risk since implants fail unexpectedly through no fault of the surgeon.

: The post op complications should be discussed in detailed with your oral surgeon, I advise. I do not see any reason that your activities are limited after you are fully recovered.

Absolutely: The dentist performing the procedure needs to feel confident that the implant being placed will have a reasonable chance of success. If the bone volume/quality is not as expected, the procedure is often terminated. This is not uncommon, and less traumatic than placing the implant only to have it fail.
