i have sores all over my mouth, but mostly on my inner upper and lower lip. they look a lot like koplik's spots, ie small white sores in large clusters surrounded by red. (i can provide a picture.) my mouth hurts and gums bleed. what's going on?

2 problems: Sores and bleeding gums. Sores may be from hermetic infection, canker sores, or other lesions. Bleeding gums probably due to bacterial infection. Please, you need to see a Dentist for proper diagnosis and treatment. Probably can't fix both problems with just a pill, poultice, or mouth rinse. You will need hands-on dental care.

Gingiva : Where the abnormality is coming from?Shape , color or something else?If gum bleeding is what you meant it needs to be evaluated by a dentist or periodontist  for checkup and cleaning or possible periodontal treatment

Apthos ulcer: Apthos ulcers do not typically occur on the gingiva so you may have something else going on.  See a dentist and have this Rea checked.

Bohns nodules: Bohn's nodules are smooth whitish bumps or cysts which are sometimes found in the mouths of newborns. They are found at the junction of the hard and soft palate, and along lingual and buccal parts of the dental ridges, away from the midline. These nodules are 1-3 mm in size, and filled with keratin.

Trauma or infection: Please see a gum specialist, a periodontist, for examination, diagnosis, and treatment plan.  May be a simple as food debris fought under the edge of the gums.  May be from systemic disease.  May be from localized infection or from the gums getting scratched.  See a pro, see a periodontist.

in mandibular gingiva tumor , why level 4 and 6 are not included in the lymph node dissections ?

Too far away: The first group of lymph nodes close to the mandible is what is removed because they are the first set to receive any cancer cells that may float away. If levels 2,3 and 4 lymph nodes are normal(free of tumors) then the more distant ones(level 5 &amp; 6) are not likely to be involved by the cancer. Hence they do not need to be removed in most cases.

  AlloDerm in the human doner tissue form seems to be highly sucessful and very predictable in gum tissue grafting. Contact a convenient Periodontist to consult in this matter. Wish you luck!

i have irritated white marginal gingiva lingually and erythmatous bulbous gingiva labially after using bleaching gel on semi-custom silicone bite tray?

Stop: Stop using trays/gel.  Return to your Dentist for reevaluation.  You are having an adverse reaction to either the gel or the trays themselves.

are gum grafts, when healed, as tight around the tooth as "normal" gingiva attachment?

Generally no: But the tighter the better.
